InnovAge offers an alternative to nursing homes through its Program of All-inclusive Care for the Elderly (PACE), providing enrolled seniors with customized healthcare and social support at PACE Adult Day Health Centers.
Responsibilities
- Provide skilled nursing care in accordance with InnovAge policies and all applicable regulations to participants.
- Assist in the examination, testing, and treatment of participants.
- Implement orders written by primary care providers.
- Triage participant and staff phone calls to determine the priority of care and route accordingly.
- Schedule outside appointments for participants when necessary.
- Administer prescribed medications and treatments.
- Provide training to participants in self-administration of applicable medications and monitor medication compliance.
- Document all necessary information and maintain participant medical record(s).
- Provide health counseling and education to participants and their families.
- Observe, record, and report participant’s condition and reaction to drugs and treatments to providers.
- Assist with clinical teaching and clinical supervision of CNA Day Center staff.
- Develop, implement, and maintain a current nursing care plan.
- Supervise LPN/LVN’s and Medical Assistants in addition to evaluating the overall effectiveness of the clinic in the absence of the Nursing Services Director/Clinic Nurse Supervisor.
- Ensure that exam rooms and/or triage area is stocked for necessary supplies and materials and that appropriate staff maintains a list of medical supplies.
- Effectively communicate and actively engage in interdisciplinary team meetings, family meetings, and clinic meetings.
- Participate within the interdisciplinary team in the formulation of Plans of Care for participants, as well as in other interdisciplinary team settings.
- May be required to provide after hours and holiday coverage.
- Comply with all regulatory and policy and procedure guidelines.
Requirements
- Current State issued Registered Nurse License.
- Current First Aid and BLS certifications are required prior to hire (acceptable vendors: American Heart Association and/or American Red Cross).
- 1+ year working with the frail or elderly.
Qualifications
- Bachelor’s degree in nursing (preferred).
- 1+ year acute care nursing (preferred).
- 3+ years health care experience with emphasis in geriatrics (preferred).
- Bi-lingual in Spanish, Russian, Punjabi, or Hmong (preferred).

Pay
Posted pay range: $41.73 - $54.28 per hour.
