Mobile Medication Monitoring Support Staff

Duties Include

• Visit consumers in their homes, the community or other locations as indicated to monitor the medications of each consumer.
• Aid in the creation and execution of a Medication Monitoring Program, including service assessments, treatment plans, and crisis intervention/recovery plans for each consumer.
• Execute medication monitoring protocols according to the needs of the consumers and the program.
• Teach and support each consumer in understanding their medications and developing independence in managing their medications.
• Preserve accurate records, complete daily service notes, and other required paperwork, and communicate with colleagues and supervisors through phone, text, and email.

Minimum Requirements

• High School diploma
• At least one year of employment experience with individuals having mental illness or intellectual disabilities, or an equivalent combination of experience and education
• Valid driver’s license and reliable transportation
