Responsibilities

  • Routine clinical care and assessment rounds at assigned communities as needed
  • Check in with community staff upon arrival to identify urgent needs or triage concerns
  • Perform focused assessments including vital signs, pain, mental status, and basic cardiopulmonary/GI observations
  • Identify skin integrity and wound status
  • Perform wound care as directed, including dressing changes and monitoring
  • Provide peripheral IV insertion, hydration, maintenance, and discontinuation with approved competencies
  • Maintain catheter function and output
  • Monitor for adverse reactions, complications, or lack of response and escalate promptly
  • Conduct follow-up check-ins after transitions or acute events in urgent cases as directed by clinical team
  • Communicate frequently and proactively with the primary care team
  • Provide non-urgent POA/family updates as directed
  • Build strong collaborative relationships with community staff, residents, and families
  • Maintain a consistent, visible presence within assigned communities
  • Partner with community leadership to identify needs and opportunities
  • Support member growth, retention, and overall satisfaction
  • Share field insights to inform care delivery and expansion efforts

Qualifications

  • Licensed Vocational Nurse (LVN)
  • Associate's Degree or equivalent experience preferred
  • 3-5 years of related experience
  • Handles basic issues and problems and refers more complex issues to higher-level staff
  • Possesses beginning to working knowledge of subject matter
