About the role

We are currently seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) Minimum Data Set (MDS) Coordinator to join our dedicated team of healthcare professionals.
Responsibilities
- Captures assessments and care plans for residents using the Resident Assessment Instrument (RAI) process in accordance with the laws, regulations, and company standards.
- Reviews and coordinates Patient Driven Payment Model (PDPM) and Case Mix data for the facility to ensure appropriate reimbursement.
- Captures and maintains schedules for all MDSs.
- Ensures accuracy of MDS data and meets with staff as needed to address any issues.
- Completes and inputs MDS data into the computer system.
- Corrects any rejected records and prepares them for re-submission.
- Formulates and implements Resident Care Plans with the interdisciplinary care plan team.
- Schedules and conducts Care Plan meetings on a regular and timely basis.
- Communicates the Care Plan to relevant staff members and reports non-compliance to the supervisor.
- Collaborates with in-house and ancillary departments to ensure compliance with the Care Plan.
- Performs other related duties as directed by the Director of Nursing Services and Administrator.
Requirements
- Current license as an RN or LPN issued by the State Board of Nursing.
- Experience with the RAI process.
- Strong knowledge of PDPM and State Case Mix Time Weighted Reimbursement payment system.
- Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long-term care.
- Effective verbal and written English communication skills, and customer service orientation.
- Strong attention to detail and accuracy, organizational skills, and the ability to prioritize and coordinate multiple projects.
- Function independently, possess flexibility, personal integrity, and the ability to collaborate effectively with employees and vendors.
- Demonstrate basic to intermediate skills in Microsoft Word, Excel, PowerPoint, and internet navigation (preferred).
- Maintain confidentiality and exhibit high levels of professionalism.
