Job Summary

The Minimum Data Set Coordinator ensures accurate and timely completion of the federally mandated MDS/RAI process for the state of South Dakota. This role is crucial for maximizing reimbursement, developing comprehensive care plans, and ensuring compliance with state and federal regulations.

Essential Functions

  • Assesses prospective residents in conjunction with other team members.
  • Aid in the development of restorative nursing programs.
  • Monitor implementation, carry through into RA flow sheets and care plans.
  • Track programs and document quarterly evaluations.
  • Complete interdisciplinary chemical review sheets for psychotropic drugs.
  • Communicate with the team and physicians as needed.
  • Accumulate and update diagnosis listings upon admission.
  • Document and follow through with care plans based on assessments.
  • Identify and implement scheduled toileting programs and bladder retraining.
  • Input MDS assessments into the Accucare system and obtain validation receipts.
  • Update and document monthly ADL flow sheets.
  • Manage the care plan process, ensuring timely meetings and updates.
  • Collaborate with the therapy department.
  • Perform other duties as assigned.

Required Additional Requirements

  • Certification - Registered Nurse (RN) - South Dakota Board of Nursing
  • Certification - Resident Assessment Coordinator-Certified (RAC-CT)
  • Certification from an accredited university or accredited training professionals

Physical Requirements

Medium work - Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
