Job Description
Plans, coordinates, manages, and directs all clinical activities and programs for home health in accordance with regulatory guidelines and professional standards.
Provides direction in the assessment, planning, implementation, and evaluation of patient care.
Maintains and interprets operational indicators to detect changes impacting staffing needs, productivity, revenue, expenses, quality indicators, patient satisfaction, and clinical and financial scorecards.
Improves outcomes through continuous improvement initiatives to ensure quality patient care is delivered cost-effectively.
Oversees the scheduling process and monitors staff productivity.
Uses audit results to develop and implement education plans to improve patient services.
Monitors the delivery of services to ensure compliance with the specificity and frequency of services ordered in the plan of care.
Oversees the quality improvement program to ensure the safe delivery of services and evaluates the quality and accuracy of clinical documentation.
Participates in compliance and record audits; troubleshoots and corrects processes for timely and accurate billing.
Interviews, hires, coaches, counsels, evaluates, and terminates personnel appropriately.
Oversees the delivery of care, ensuring patients meet established criteria to initiate and discontinue homecare services.
Affirms that adequate qualified staff are available to meet the needs of the agency.
Performs other duties as assigned.
Knowledge, Skills, And Abilities
- Ability to communicate effectively in English, both verbally and in writing.
- Comprehensive knowledge of state and federal home care regulations.
- Demonstrates moderate to proficient computer skills, including familiarity with electronic medical records for Home Health and proficiency in Microsoft Office applications (Excel, PowerPoint, etc.).
- Excellent organizational skills with a keen attention to detail, capable of coordinating meetings, spearheading projects, and presenting information effectively to various audiences.
- Interpersonal skills necessary to provide effective leadership, develop and maintain cooperative working relationships with diverse Hospital departments, and interact positively with the public and staff at all levels.
- Ability to work independently with minimal supervision and demonstrate a broad clinical background, preferably with 5 or more years of clinical experience in medical/surgical or critical care.
- Current knowledge of homecare requirements and OASIS assessment.
Education
- Associate's [Required]
- Bachelor's [Preferred]
- Field Of Study in Nursing or other healthcare related field.
Work Experience
- 1+ year of supervisory or administrative experience in medicare certified home care [Required]
- Home health experience within the past 3 years [Preferred]

Pay Range
$72,786.83 - $135,385.27
