Mental Professional I/Assertive Community Treatment (ACT) Team - (Lexington)

Job Responsibilities

Assertive Community Treatment (ACT):

• Serves on the ACT team providing evidence-based services.
• Guides patients in developing skills or strategies toward achieving and maintaining good mental health.
• Utilizes assertive engagement to engage beneficiaries in service.
• Affords patients access to community services, resources, and agencies and establishes support networks in the community.
• Works with beneficiaries' natural supports.
• Participates in the person-centered planning process for patients of the ACT program.
• Attends treatment teams meetings daily and provides consultation regarding complex cases as appropriate.
• Completes documentation according to QA/SCBHDD standards, complies with Corporate Compliance Policy and Risk Management Plan, and ensures timely completion of prior authorizations.
• Completes all annual required trainings to include Safety, MAPS, Cultural Affairs, and clinical trainings.
• Demonstrates adherence to CARF standards, and SCBHDD and Center policies and procedures.

Minimum and Additional Requirements

• Licensed as an LISW-CP, LMFT, or LPC in the state of South Carolina.
• A master's or doctoral degree from a program that is primarily psychological in nature from an accredited university.
• One year of experience working with adults with serious mental illness.
• Ability to lift 5lbs. and operate a state vehicle.
• Flexible schedule.

Preferred Qualifications

• Bilingual abilities in English and Spanish (or another language).
