CMMHC has a rewarding opportunity for a Mental Health Professional or Clinical Trainee to serve as Team Lead for our St. Cloud Assertive Community Treatment (ACT) team. As the ACT Team Lead, you'll provide clinical leadership to a collaborative team while staying connected to the people we serve and the realities of community-based care. ACT brings a multidisciplinary team into the community to support people with serious and persistent mental health needs, working together to share responsibility for clients and respond to changing needs.

About the role

As ACT Team Lead, you'll help ensure the team delivers services consistent with the PACT Model, Minnesota Statutes, the Tool for Measurement of Assertive Community Treatment (TMACT), and other evidence-based ACT practices. This role involves leading a team in community-based settings rather than traditional office-based care, focusing on person-centered support and recovery.

Responsibilities

  • Lead as a team, promoting shared responsibility, strong communication, and collaborative decision-making across the multidisciplinary ACT team.
  • Support community-based services in clients' homes, neighborhoods, and other community settings.
  • Provide clinical leadership as the team responds to psychiatric symptoms, crises, hospitalizations, housing instability, substance use, medical concerns, and other challenges.
  • Help clients build greater stability, independence, meaningful relationships, employment, housing, and community connections while respecting their goals, choices, and autonomy.
  • Assess, refer, coordinate, and monitor services while communicating with providers and community partners to reduce duplication and close gaps in care.
  • Maintain current knowledge of mental health, medical, housing, employment, social service, and community resources and connect clients with those resources.
  • Maintain overall clinical oversight of the ACT team after regular business hours, on weekends, and holidays, delegating when appropriate.
  • Maintain optimal staffing levels, participate in hiring, oversee staff training, provide coaching, and conduct performance evaluations.
  • Support the financial viability of the program and provide planning and operational leadership to secure necessary resources.

Requirements

  • Licensed as an LP, LPCC, LICSW, or LMFT, or a Clinical Trainee actively working toward licensure.
  • Clear understanding of the Assertive Community Treatment service delivery model.
  • Ability to provide clinical leadership and make sound decisions in complex and changing situations.
  • Ability to work effectively as part of a multidisciplinary team.
  • Successful completion of a DHS background study.
  • Valid driver's license and acceptable driving record.

Qualifications

  • Two years of supervisory, program management, or leadership experience strongly preferred.
  • Experience working with individuals with serious and persistent mental illness.
  • Experience with Assertive Community Treatment, PACT, or another intensive community-based service model.
  • Experience providing services in community settings rather than exclusively in an office.
  • Strong collaborative leadership and communication skills.
  • Ability to balance clinical judgment, team leadership, and a recovery-oriented approach.
  • CPR and First Aid certification preferred.

Pay

Compensation target range: $75,000 - $90,000.

Schedule

  • Monday-Friday, 7:30 am-3:30 pm.
  • Rotating weeks of after-hours/weekends clinical coverage as part of the ACT team.
