Requirements
- Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or another behavioral science and experience working with adults with serious mental illness (SMI)
- Bachelor's degree in a non-behavioral science field with experience supporting adults with serious mental illness or individuals with similar human service needs
- High school diploma or equivalent with relevant experience working with adults with serious mental illness or individuals with similar human service needs
- Related training or certifications (e.g., Certified Occupational Therapy Assistant, Home Health Aide) are preferred
- Experience in human services, healthcare, education, or other helping professions is a plus
- Strong interpersonal, communication, and relationship-building skills
- Ability to provide rehabilitation, recovery-oriented support, and community-based services as part of a multidisciplinary team
- Valid PA Driver's License
Qualifications
- Assist in providing rehabilitation and support services for individuals with mental health and behavioral health diagnoses
- Support treatment plans, including substance abuse services, education, and family consultation, under clinical supervision
- Provide crisis intervention services as needed
- Collaborate with families to offer education, support, and guidance
- Collaborate with various community resources, including healthcare providers, social services, housing agencies, and other support organizations, to ensure comprehensive care for clients
- Ensure the safety and well-being of consumers in residential programs by adhering to safety protocols
- Participate in direct care responsibilities, including daily living support and therapeutic activities
- Follow and contribute to the treatment planning process in collaboration with the clinical team

About the role
At Merakey, we put heart and soul into everything we do. We are seeking a Case Manager to join our team at our program in Harrisburg, PA location supporting Dauphin County. Earn a $21.01 an hour + mileage reimbursement.
Position Details
