About the role
Mental Health Workers within the Involuntary Outpatient Commitment (IOC) program at RWJBarnabas Health offer support to adults with severe and persistent mental illness. This role focuses on comprehensive case management, care coordination, and promoting recovery and community stability.
Responsibilities
- Provide comprehensive case management services to adults with severe and persistent mental illness enrolled in the Involuntary Outpatient Commitment (IOC) program
- Conduct ongoing assessments of consumers throughout program enrollment to identify needs, monitor progress, and support recovery goals
- Develop and update individualized, person-centered rehabilitation plans in collaboration with consumers, families, and the interdisciplinary care team
- Coordinate and facilitate access to behavioral health treatment, medical care, housing, transportation, benefits, and other community resources
- Deliver care coordination, health promotion, supportive counseling, crisis intervention, and transitional care services to promote stability in the community
- Assist consumers with treatment enrollment, establishing community supports, and connecting them with social service agencies and recovery resources
- Collaborate closely with psychiatrists, nurses, social workers, family members, and community providers to ensure coordinated, consumer-centered care
- Maintain accurate, timely documentation that meets billing, regulatory, and organizational standards
- Provide transportation for consumers as needed to support access to treatment and essential services
- Support the department's mission of helping consumers achieve recovery, maintain community stability, and progress toward greater independent living
Qualifications
- Education: Bachelor's degree in Psychology, Social Work, Counseling, or another mental health-related field required
- License: Valid New Jersey driver's license required
- Language: Bilingual Spanish preferred, but not required
- Experience: Previous experience in behavioral health, psychiatric services, case management, community mental health, or human services is preferred
- Skills: Strong interpersonal, communication, crisis intervention, care coordination, and documentation skills, with the ability to work effectively as part of a multidisciplinary team
