About the Role

The Integrated Behavioral Health Therapist independently provides assessments, treatment/care planning, and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care, and chronic disease management patients. The role involves using a variety of theoretical treatment models, evidence-based practices, skill-building, and interventions.

Responsibilities

  • Implement mental health assessments and utilize the DSM-5 to provide accurate diagnoses/differential diagnoses.
  • Develop person-centered treatment plans based on assessments.
  • Employ therapeutic interventions to address identified concerns and support needs.
  • Document individual and family sessions, using the client treatment plan to increase engagement and ownership.
  • Document communication with primary care clinic providers and outside support resources.
  • Collaborate and maintain positive relationships with primary care team members and outside resources to facilitate quality patient care.
  • Meet facility productivity and documentation standards.
  • Participate in quarterly care team meetings and other meetings as requested by clinic leadership.
  • Maintain reliable attendance.
  • Contribute to organizational success by meeting competency expectations and core values (respect, integrity, stewardship, excellence, collaboration, and kindness).
  • Practice proper fire, safety, and disaster procedures as set forth by the organization.
  • Participate in mandatory safety, security, and infection control programs.
  • Uphold confidentiality and HIPAA compliance; report violations to the Privacy Officer.
  • Comply with the organization’s Compliance Program, including the Code of Conduct and all relevant policies.
  • Participate in compliance-related education and training programs.
  • Demonstrate commitment, motivation, professionalism, and ability to work independently.
  • Perform other duties as assigned.

Requirements

  • Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or a related field.
  • 2 years of experience providing direct therapy is preferred.
  • Current licensure as a Licensed Clinical Social Worker (PCMSW, CMSW) or Licensed Mental Health Practitioner (PLMPH, LMHP, LIMHP). Provisional status may be considered.
  • Ability to read, write, and follow written and oral instructions with strong communication skills.
  • Ability to calculate at the level necessary to perform essential duties.
  • Ability to make independent decisions and interpret instructions in various forms (oral, written, diagram, or schedule).

  • Frequent sitting and long periods examining documents.
  • Standing, reaching with arms and hands, and occasionally lifting, pushing, or pulling (weight not specified).
