Mental Health Therapist

Responsibilities

• Provide evidence-based outpatient therapy tailored to diverse patient needs.
• Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
• Collaborate with psychiatrists, primary care providers, and care managers to coordinate holistic care and ensure continuity of services.
• Support crisis intervention and maintain accurate clinical documentation in the EMR.
• Participate in case conferences, quality-improvement initiatives, and patient education programs.

Qualifications & Skills

• Licensed Mental Health Therapist with relevant licensure and credentials.
• Strong clinical skills in outpatient mental health therapy and psychosocial assessment.
• Ability to work collaboratively within a multidisciplinary team.
• Excellent documentation and communication skills.
• Commitment to quality improvement and patient-centered care.
