Job Summary
RADIAS Health is hiring a Mental Health Practitioner Supervisor to lead a group home with four residents. The supervisor will oversee care coordination, client medical appointments, and staff supervision.
Responsibilities
- Coordinate client care and establish quarterly case conferences with the case manager, treatment team, and family members.
- Contact family members, therapists, or case managers as needed.
- Implement and review quarterly Individual Service Plans, Individual Abuse Prevention Plans, Emergency Service Plans, and behavior plans.
- Schedule and coordinate client medical appointments and transportation.
- Develop additional programming for clients with high service needs and acute symptoms.
- Aid residents in independent living skills, including hygiene, budgeting, and completing financial forms.
- Assist with client admission and discharge processes, connecting them to community resources.
- Lead weekly staff meetings and distribute meeting minutes.
- Administer medications according to policy and ensure completion of monthly pharmacy orders.
- Complete incident reports and perform client record audits.
- Oversee recreational and social activities, holiday events, and client transport.
Requirements
- Associate's degree in human services with 2 years of full-time work experience in care or education to vulnerable adults or children.
- Bachelor's degree in human services with 1 year of full-time work experience in care or education to vulnerable adults or children.
- Valid driver's license, auto insurance, and access to a vehicle.
