Starting Compensation

24.56 - 26.37 USD Per Hour

Qualifications

• Education:
  • Facilitator II: Master's degree in Mental Health, Human Services, or closely related field
  • Facilitator I: Bachelor's degree in Mental Health, Human Services, or closely related field
• Experience:
  • Facilitator II: One (1) year of experience in a mental health field, and direct service experience with severely emotionally disturbed children, adolescents, and families; experience with high-risk children and families
  • Facilitator I: Two (2) years of experience in a mental health field, and direct service experience with severely emotionally disturbed children, adolescents, and families; experience with high-risk children and families
• Licensure & Certification:
  • Valid California Driver’s License and maintenance of a driving record that meets the company driver eligibility policy
  • CANS Certification within 30-days of hire
  • Completion of Statewide Standardized Child and Family Team (CFT) Facilitation Training within one (1) year of hire

Responsibilities

This position has primary responsibility for coordinating services across various programs, including:

• Engagement, assessment, Child and Family Team (CFT) formation and implementation
• Development of Safety Crises Plan and Strengths Summary
• Providing a broad range of case management services meant to stabilize and reduce involuntary hospitalization and/or placement disruption
• When indicated as a need by the youth/adult/family, providing mental health services such as individual rehabilitation, collateral intervention, and coaching
• Acting as a liaison with external agencies and convening the Child and Family Team
• Ensuring the team consists of family members, County representatives, the client, and others selected by the family
• Working to ensure all team members are active participants in the CFT Process
• Working with family to generate Mission statement and list of underlying needs, while assisting child/family and team to identify and prioritize needs
• Collaborating with team members to ensure adherence to program process and principles, as well as, TEAMMATES philosophy of “Whatever It Takes”

Division/Program Overview

• Programs: High Fidelity Wraparound services: Comprehensive Behavioral & Social Support Services, Crisis response, team-based approach
• Population Served: Children, Youth, and Families battling trauma, mental illness, neglect, or substance abuse
• Facility & Setting: Multidisciplinary program sites serving SPA 1,2,3,4,6,7,8
• Clinical Services: Trauma-informed care, mental health treatment, behavioral interventions, and parental skill enhancement
• Length of Stay: Based on need, average stay is 18 months
• Skills & Transition: Strengths-based empowerment, resilience building, and long-term recovery/re-integration support; Family support, skill development, advocacy, and comprehensive community resources
