Responsibilities

  • Providing quality clinical therapeutic services to patient populations and their families/significant others.
  • Serving as a member of the interdisciplinary team, supporting the organization's treatment program and philosophy and assuring the deliverance of quality treatment.
  • Conducting psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus.
  • Contacting appropriate previous and current tertiary caregivers to obtain relevant collateral clinical data that may be needed for the patient’s coordination of care, treatment planning and discharge planning.
  • Developing, implementing and monitoring progress of the patient’s individualized treatment plan.
  • Conducting daily group and individual assessments to identify issues for treatment focus by interviewing the patient one-on-one and transcribing relevant information.
  • Providing individual, group and family process-oriented counseling sessions.
  • Conducting educational, after care support and other didactic groups for patients and family members using various professional treatment modalities.
  • Participating in regular clinical team meetings/staffing and providing input on the psychosocial, disease management and case management needs of the patient.
  • Demonstrating an understanding, and adhering to all procedures and legislation, which apply to patients cared for in a Mental Health and Substance Abuse treatment facility.
  • Maintaining accurate and timely medical records including psycho-social assessments, family session and daily progress notes to include school personnel and other community collateral involvement.
  • Reporting to community/state agencies any patient issues that are required by hospital policy and state or federal law. Serving as the facilities representative in legal proceedings resulting from said reports.

  • Demonstrates conduct in keeping with LOH ethical standards.
  • Complies with facility safety, infection control and security program.
  • Floats to alternate units or Linden Oaks locations based on census and staffing needs.

Qualifications

  • Minimum of two years of experience in a behavioral health setting.
  • Certified in CPR and CPI (Crisis Prevention Intervention).
  • Preferred: CADC when primary patient population is chemical dependency.

Skills

  • Knowledge of developmental psychology, disease pathology and clinical needs of assigned population.
  • Ability to respond appropriately to the physical and psychological needs of the patients.
