Mental Health Caseworker/QMHP (ACT Team)

Responsibilities

• Assisting individuals with accessing medical, social, educational, and other needed resources
• Providing rehabilitative and skills training
• Performing case management and care coordination
• Developing and implementing treatment plans
• Offering crisis services
• Documenting client progress
• Providing supported employment services
• Offering telemedicine services

Requirements

• A valid Texas driver's license and an acceptable driving record
• Personal liability insurance as required by the state of Texas
• Working computer skills including Microsoft Office
