Essential Functions
- Conducts daily visits in the community, and subsequent progress notes and other required documentation in accordance with agency, ACT, and OMH standards that demonstrate evidence-based practices
- Completes and maintains shared assigned charts such as initial assessments including Initial Service Plans and initial Comprehensive Assessments, Wellness and Safety Plans, update Service Plan Reviews with Comprehensive assessments every 6-months using evidence-based practices and assessments required, risk assessments and other documentation as required
- Completes individual psychotherapy with consumers using evidence-based treatment modalities
- Participates in daily morning meeting, and informs ACT team in daily meetings and weekly clinical meetings around family issues and incorporate findings into service plans
- Involves family and significant others in the consumer’s treatment with the ACT team and provide psycho-education and relapse prevention based on motivational interviewing and cognitive behavioral strategies during visits
- Act as advocate and liaison for consumers in relationships with family, support systems, landlords and other community supports
- Provide 24-hour crisis intervention on-call services on a rotating basis
- Performs other duties, consistent with the goals and objectives of the program, as directed by Team Leader
Qualifications
- Master’s Degree required – MHC-LP, LMHC, LMSW, LCSW, or equivalent in licensure in NYS preferred
- At least one year of experience working in human services with individuals with serious mental illness and experience in work with families
- Must be outcome oriented in a fast-paced environment with deadlines
