Mental Health Case Manager
About the role
As a Case Manager within our Mental Health team, you'll play a vital role in supporting individuals with a mental health diagnosis. This position is ideal for someone who is empathetic and thrives in fast-paced environments where collaboration matters.
Core Responsibilities
- Conduct assessments to identify individual needs, strengths, and goals
- Develop and monitor individualized service plans in partnership with individuals and their supports
- Cookordinate access to mental health services, supports, and community resources
- Respond to crises and provide timely intervention and support
- Maintain accurate, timely documentation in accordance with confidentiality and compliance standards
- Engage individuals in recovery-oriented planning, promote self-sufficiency, and provide advocacy
- Participate in team meetings, training, and supervision
- Occasional on-call and weekend hours required
Who You Bring
- Bachelor’s Degree in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or a related field; OR 12 credits in a human services field and 2 years of related experience
- Experience in mental health or developmental disability services preferred
- Ability to work independently and as part of a team
- A valid driver’s license and reliable transportation

Location & Schedule
This position is based in Lycoming and Clinton counties, PA. Work is primarily community based. Office hours are Monday through Friday, 8:30 AM to 4:30PM. Mental Health Case Managers have on-call responsibilities.
