Mental Health Case Manager

Responsibilities

  • Aid in the completion of a comprehensive assessment and treatment plan for each client.
  • Affirm that all clients on the assigned caseload receive an environmental matrix to identify skill deficits and strengths to establish treatment goals.
  • Create and implement a written service plan for each client.
  • Offer services to diffuse or resolve crises to prevent unnecessary use of emergency rooms, in-patient services, and mental health court.
  • Connect clients to behavioral health and social service agencies to ensure there are no gaps in service.
  • Mak home, school, hospital, and community-based visits on a daily basis. Must be able to travel throughout Bucks County to meet face to face with clients, families, schools, providers, etc., as needed.
  • Utilize IPAD and cell phone provided to complete documentation tasks and outreach to clients, providers, coordinate resources, etc.
  • Be familiar with and able to access community resources and child-serving systems in Bucks County.
  • Participate in weekly team meetings and supervision sessions.
  • Achieve billable hour requirements per week.
  • Assume a case load of up to 30 clients.
  • Provide on-call support for 24/7 mental health crisis support in a rotation with other program staff.
  • Implement the 5 core values of Trauma-Informed Care in all work practices - Safety, Trustworthiness, Choice, Collaboration, & Empowerment.

Requirements

  • 12 credits in coursework related to Human Services, Psychology, or related field
  • One or more years of direct clinical experience working with children, youth, and families
  • Manage a varied work schedule Monday-Friday between 8:30am - 8:30pm, including 3 required evenings per week

Qualifications

  • Bilingual in Spanish (preferred)
