Mental Health Case Manager

We are seeking a compassionate Frontier Mental Health Case Manager to support individuals and families across West Texas.

Responsibilities

• Assessing client needs and developing personalized service plans
• Coordinating behavioral health and community resources
• Monitoring client progress toward recovery and independence
• Collaborating with multidisciplinary teams and community partners
• Maintaining accurate documentation and advocating for clients
• Supporting clients navigating mental health, substance use, and crisis services

Qualifications

• Bachelor's degree in social work, psychology, or a related field (preferred)
• Relevant case management experience
• Strong communication, organizational, and advocacy skills
• Ability to work effectively within a community-focused, mission-driven environment
