About the role

The Mental Health Advocate will work in collaboration with the consumer on goals addressed in individual recovery plans to maximize the consumer's successful community adaptation and quality of life.

Responsibilities

  • Provide direct services to consumers and their families; relapse prevention, daily living skills including money management; development and support of social and recreational activities; consumer psycho education and support family and significant others education and support, consumer advocacy, crisis intervention, securing and maintaining housing and financial resources; transportation for consumers in agency vehicles with adherence to agency policies and procedures, etc
  • Coordinate with other services provided to consumers.
  • Participate in an interdisciplinary team process to ensure the provision of successful implementation of individual recovery plan goals.
  • Record, report and monitor service delivery as per program contract requirements.
  • Attend required annual trainings mandated by DMHS standards.
  • Understand and participate in successful completion of fidelity scale goals.
  • Liaison with payee provider - family and children services.
  • Aid clinicians, vocational specialists, co-occurring specialists to facilitate groups.
  • Aid nurses in securing medical appointments and lab work.

Qualifications

  • High School diploma or GED
  • BA Degree with two years' experience in related field (Preferred)

Physical Requirements

  • Prolonged sitting, some bending and stretching
  • Good eye-hand coordination and manual dexterity sufficient to operate standard office equipment
  • Normal range of hearing and eyesight to record, prepare, and communicate information pertaining to this position
  • Occasional lifting and/or moving objects up to 30 pounds
