About the role

Assists the Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person-centered, dementia-friendly programs that meet the specific needs of each resident within the dementia-care community. In alignment with Brookdale’s Resident and Family Engagement philosophy, you will help provide person-centered programs that engage residents in meaningful and purposeful dementia-care activities, enhancing their quality of life while honoring individual abilities, historical routines, interests, and preferences.

Responsibilities

  • Supports the implementation of person-centered programs tailored to residents’ needs and interests.
  • Assists in leading and engaging residents in the 7 domains of well-being programs—purposeful, intellectual, social, spiritual, emotional, and physical—in a dementia-friendly, structured environment; includes facilitating sensory-focused small groups for residents who cannot participate in large-group settings.
  • Assists the Program Manager or Coordinator in reviewing individual resident service plans and completes program documentation as required by Brookdale standards or state regulations.
  • Ensures dementia-care programs comply with state, federal, and other regulations and align with Brookdale’s mission, policies, procedures, and standards.
  • Provides guidance and resources to associates on the importance of programming/engagement and other areas regarding residents’ quality of life.
  • Coordinates and participates in special events at the community, including family socials, marketing/sales events, and open houses.
  • Communicates positively with residents, families, and community professionals about the Brookdale dementia-care program and its benefits; shares residents’ personal needs and successes with families.
  • Assists the Manager or Coordinator with meeting the department’s quality-improvement initiatives, including the Brookdale Excellence Standards Tool (BEST).
  • Supports planning and scheduling programming events outside the community, which may include driving a community motor vehicle.
  • Assists with the volunteer program, including training and supervising volunteers.
  • Performs personal care, housekeeping, and dietary responsibilities as needed to maintain service quality in accordance with Clare Bridge Program standards.

Requirements

  • High School Diploma or equivalent required; Associate Degree preferred.
  • Minimum one year of previous experience with older adults and persons with dementia preferred, or an equivalent combination of education and experience.
  • Must have a valid driver’s license and a current 3-year driving abstract with no more than one moving violation as a condition of employment.

Skills

  • Basic technical skills to complete repetitive, well-defined duties.
  • Basic knowledge of the organization to answer routine questions.
  • Basic PC and word-processing skills, preferably in a Microsoft Windows environment.
  • Effective oral and written communication skills.

Physical Demands

  • Standing, walking, sitting.
  • Use of hands and fingers to handle or feel; reaching with hands and arms.
  • Climbing or balancing; stooping, kneeling, crouching, or crawling.
  • Talking or hearing; ability to lift up to 50 pounds.
  • Vision.
  • Interaction with co-workers, residents, or vendors.
  • Occasional weekend, evening, or night work if needed for shift coverage.
  • On-call as needed.
  • Possible exposure to communicable diseases, infections, latex, blood-borne pathogens, and various drugs, chemicals, or biological hazards.
  • Occasional travel required.

Pay

Base pay in range will be determined by applicant’s skills and experience.
