Memory Care Manager
About the Role
The Memory Care Manager at Pillars of Prospect Park is accountable for implementing the Dimensions Program, encouraging participation and ownership from all staff. This role manages and supervises the programmatic, physical, and educational operations of the Dimensions Community to meet resident and family needs and expectations. You will collaborate with the interdisciplinary team and provide education to staff and families to equip them to interact effectively with residents experiencing dementia.
Responsibilities
- Leadership & Staff Development:
- Provide leadership, supervision, and ongoing support to staff, volunteers, and students.
- Partner with the Director of Health Services (DHS) to support staffing, employee onboarding, training, development, and performance management.
- Coach and mentor team members on dementia care best practices and resident engagement.
- Assist with hiring and training activity staff.
- Dimensions Program Leadership:
- Serve as the site's Dimensions Champion and maintain required Dimensions Program training and certification.
- Implement, manage, evaluate, and continuously improve the Dimensions Program.
- Ensure the Dimensions environment promotes engagement, well-being, and a positive resident experience.
- Facilitate care partner support groups and family education opportunities.
- Resident Programming & Engagement:
- Assess resident interests, strengths, and psychosocial needs to develop meaningful programming.
- Plan, coordinate, and oversee daily activities, special events, and community outings.
- Develop monthly activity calendars in collaboration with the Activities Director.
- Encourage resident participation while supporting independence and quality of life.
- Ensure adequate supplies and resources are available to support programming.
- Resident Care Collaboration:
- Collaborate with nursing and interdisciplinary teams to incorporate cognitive assessments, care plans, and individualized approaches to care.
- Promote the use of non-pharmacological interventions and person-centered dementia care practices.
- Support residents in maintaining their highest possible level of functioning and independence.
- Customer Service & Family Relations:
- Build positive relationships with residents, families, staff, and community partners.
- Address concerns promptly and effectively.
- Review satisfaction feedback and implement improvements as needed.
- Communicate program updates through family newsletters and other channels.
- Operations, Marketing & Administration:
- Maintain accurate program, attendance, and departmental records.
- Partner with leadership on budgeting, financial oversight, occupancy goals, and community marketing efforts.
- Support resident admissions, tours, and community outreach activities.
- Ensure marketing materials remain current and effective.
- Transportation & Safety:
- Safely transport residents for outings and community activities when required.
- Complete required vehicle inspections, transportation documentation, and safety checks.
- Ensure compliance with transportation regulations, infection control standards, and organizational safety policies.
- Respond appropriately to safety concerns, incidents, and emergencies.
Requirements
- A.A./A.S. Degree
- Less than 1 year of experience
- Valid driver’s license in good standing
- Must be insurable under the employer’s/community’s auto insurance coverage
- Medical exam certification
- Must be at least 18 years old
- Must complete and pass in-house transportation curriculum course and driving evaluation within 30 days
Preferred Qualifications
- Bachelor of Arts degree
- 1 year of experience
Schedule
- Full-time: 80 hours every two weeks
- Monday–Friday, 9:00 AM–5:00 PM
- Manager on Duty weekend rotation

Pay
An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. If your role is eligible for a sign-on bonus, the bonus program approved and in place at the time of offer will be honored.
