About the role

The Harmony Square Director (HSD) Clinical reports directly to the Executive Director (ED) and is responsible for the overall leadership and daily operations of the Harmony Square neighborhood. This role focuses on creating and maintaining a safe, engaging, and person-centered environment for residents while ensuring clinical compliance with all applicable regulatory standards. The HSD actively oversees resident safety, behavior expressions, routines, and support with activities of daily living (ADLs).

Responsibilities

  • Evaluates residents prior to move-in to determine whether their care and service needs can be met in the community; conducts re-evaluations as indicated by residents’ condition and/or State regulations.
  • Develops a service plan unique to each resident based on the evaluation and the resident’s stated preferences; revises the service plan as indicated by residents’ condition and/or State regulations.
  • Collaborates with each resident’s physician, pharmacist, third-party healthcare providers, and other members of the healthcare team to promote coordinated care and services.
  • Oversees the community’s Medication Management Oversight Program; collaborates with the pharmacy, conducts medication technician training and skills validation (where applicable by state regulations), and performs routine audits to promote safety in medication administration.
  • Maintains a working knowledge of all applicable local, State, and Federal regulations; ensures resident evaluations and documentation meet all requirements and collaborates with the Healthcare Director to keep the resident care department survey-ready.
  • Conducts interviews, orientation, coaching, and ongoing training of Harmony Square team members in accordance with associate training standards and regulatory guidelines.
  • Provides supervision and direction to Harmony Square team members in collaboration with the Healthcare Director to ensure consistent, high-quality care and support.
  • Collaborates with the Life Enrichment Director to ensure the delivery of purposeful daily programming within the community and through external outings and events; leads a minimum of 1 high-quality, innovative program per month.
  • Plans, leads, and facilitates monthly Family Council Meetings to foster open communication, share updates, and strengthen family engagement.
  • Partners with the Alzheimer’s Association or other local organizations to schedule, organize, and participate in monthly Support Group meetings.
  • Utilizes and connects with local community resources; maintains the Behavior Expression process.
  • Ensures all aspects of dining are met through partnership with the Director of Dining and Healthcare Director.
  • Completes daily and bi-weekly neighborhood rounds; leads and organizes daily huddles.
  • Promotes a culture of teamwork and team member appreciation; establishes and maintains positive working relationships with all departments.
  • Effectively communicates with resident families by responding to questions and concerns promptly.
  • Completes all required courses in adherence with state regulations and Relias Learning.
  • Demonstrates passion for working with individuals with dementia.
  • Carries out other duties as assigned.

Requirements

  • Current valid and unencumbered license to practice as an RN or LPN in the State in which the community is located.
  • Experience in caring for geriatric individuals and those with Alzheimer’s and other forms of dementia strongly preferred.
  • 1-2 years’ experience required; 3 or more years’ experience preferred.
  • Current CPR certification required; dementia training or Certified Dementia Practitioner preferred or eligible for certification.
  • Strong organizational and time management skills; effective verbal and written communication skills.
  • Ability to work a flexible schedule including day, evening, and night shifts, as well as weekends.
  • Ability to be on-call after hours and act as the Manager-on-Duty on selected weekends.
  • Experience with Behavior Expressions redirection and interventions.
  • Valid driver’s license and a good driving record.
  • Good physical condition; ability to sit, stand, bend, kneel, walk, and lift up to 50 lbs.
