About the role

The Memory Care Coordinator will provide individualized, person-centered care coordination and operational support for the Memory Support Center while ensuring residents experience the highest possible physical, emotional, social, and psychosocial well-being. Coordinate clinical and administrative processes, support regulatory compliance, oversee documentation, collaborate with interdisciplinary teams, and champion the Halcyon philosophy of care.

  • Understand the importance of resident rights in healthcare as well as the community as a whole and advocate for operations, processes, and decisions that are in resident’s best interests.
  • Coordinate resident admissions, assessments, service planning, care conferences, and quarterly physician orders.
  • Review resident documentation, alerts, behaviors, weights, vital signs, and other clinical information to identify changes requiring follow-up.
  • Maintain accurate resident records, forms, MAR/TAR documentation, and computerized systems.
  • Communicate proactively with residents, families, healthcare providers, and interdisciplinary team members to coordinate care and resolve concerns.
  • Provide operational support by answering phones, coordinating appointments and transportation, and maintaining office workflows.
  • Assist with quality assurance, regulatory compliance, infection prevention, and environmental rounds.
  • Support orientation, education, and ongoing coaching of Memory Support staff while reinforcing the Halcyon philosophy of care.
  • Assist with scheduling, work assignments, and day-to-day coordination of Memory Support operations.
  • Promote resident choice, dignity, privacy, and independence in all interactions.
  • Participate in quality improvement initiatives, policy implementation, and assigned committees.
  • Provide on-call support as assigned and perform other duties as needed.

Requirements

  • Minimum two years of experience working with older adults, preferably in memory care or long-term care.
  • Knowledge of dementia care, Residential Care Facility regulations, and person-centered care practices.
  • Current CPR and First Aid certification (or ability to obtain).
  • Strong written, verbal, organizational, and computer skills.
  • Ability to prioritize, exercise sound judgment, and work independently while collaborating with interdisciplinary teams.
  • Food Handler's Card and ability to satisfy criminal background requirements, as applicable.

Qualifications

  • High school diploma or equivalent.
  • Valid driver’s license and reliable transportation.

Skills

  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong attention to detail and organizational skills.
  • Proficiency in Microsoft Office Suite.
