About the role

At Wayspring, we're offering the opportunity to make a difference by supporting individuals through their recovery journey. As a Member Engagement Specialist, you’ll be responsible for the initial engagement and enrollment of eligible health plan members within a treatment facility or community-based setting. You’ll continue to support and engage members throughout their program, ensuring they have the resources they need to succeed. This role requires regular daytime travel to meet members in and around the greater Eastern Iowa area, with mileage reimbursement included.

Responsibilities

  • Facilitate in-person enrollments at Wayspring partnered facilities, and conduct outreach to providers to secure member referrals where appropriate.
  • Engage members and provide an educational overview of Wayspring’s service offerings, with the goal of program enrollment.
  • Secure verbal and written consent, and obtain appropriate paperwork for services, including consent to clinic services and release of information forms.
  • Collaborate with facility discharge planning teams to ensure members leave facilities with a safe and stable plan.
  • Secure members' first appointment with clinical services at Wayspring.
  • Provide follow-up to members for 30-90 days post-discharge at partnered facilities, driven by individual member needs.
  • Initiate creative strategies to facilitate member contact, including in-person opportunities.
  • Provide ongoing engagement to members both telephonically and in-person.
  • Incorporate recovery support themes into engagement to improve member experience.
  • Regularly review member profiles for Social Determinants of Health (SDoH) and clinical updates.
  • Maintain performance accountability around member enrollment, clinic conversion, and achieve performance targets.
  • Adhere to Wayspring information security and privacy requirements.

Requirements

  • Minimum of three (3) years of experience in an outreach, social work, or street outreach role.
  • Highly organized, detail-oriented, and self-driven with the ability to work independently and as part of a team.
  • Demonstrated ability working in a matrixed environment.
  • Strong communication skills, including de-escalation and crisis management.
  • Exceptional verbal and written communication skills, and effective presentation skills.
  • Experience with CRM platforms or Electronic Health Record systems (EHR/EMR) is highly preferred.
