Full-time position (1.0 FTE) in the Pharmacy department at UnityPoint Health-St Luke's, working 40 hours per week with shifts available 7 days per week from 8 AM to 4:30 PM.

About the role

Under the supervision of the Pharmacist, the Medication Reconciliation Specialist is responsible for obtaining a current and accurate home medication record for newly admitted inpatients. This information is gathered from the patient, family, home pharmacy, physician office, or other facilities as required. The role involves inputting correct home medication details—including dose, frequency, time of last dose, home pharmacy, and detailed allergy information—into Epic.

Medication Historians will complete at least 14 histories per shift and will also perform reconciliation for patients requested by the Hospitalist Service.

Responsibilities

  • Obtain names of newly admitted patients going to 4th or 5th floor via ER Track Board and/or Nursing Bed Manager, and upon request by Nursing/Hospitalists.
  • Interview newly admitted patients or their families to obtain the most accurate and current list of home medications, ensuring a positive and safe environment to gather the best medication information.
  • Contact the patient’s family, nursing facility (to obtain MAG or MAR), home pharmacy, and/or physician office for medication clarifications as necessary.
  • Input the best and most accurate obtained home medication list into Epic.
  • Obtain and input into Epic information on the patient’s primary home pharmacy and any secondary pharmacies they may use.
  • Obtain and input into Epic a detailed description of drug allergies and the reactions they cause (e.g., allergy to morphine causing shortness of breath).
  • Obtain current Pneumococcal and Flu vaccine status and communicate with staff if the patient is deficient in immunizations. Document any herbal and over-the-counter medications routinely taken.
  • Demonstrate competent computer skills in Epic and strong interviewing abilities, using probing tactics to gather the most accurate medication information from patients and families.
  • Assist with audits and analyze data to promote quality care in nursing departments, aligning with medication reconciliation, pain management documentation, and medication sign-off in Omni and Epic.

Requirements

  • High School Diploma or Equivalent required.
  • 2 years of related work experience or equivalent education to replace work experience.
  • Certified Pharmacy Technician Certification required.
  • Registration with the State of Iowa Board of Pharmacy required.
