Summary
The Pharmacy Medication History Technician reports to the Manager, Pharmacy Operations, under the supervision of a Pharmacist and the direction of the Medication History Pharmacy Technician Group Leader. This role serves as a key part of the hospital’s medication reconciliation practice model, focusing on obtaining and compiling accurate home medication histories and allergies for patients.Responsibilities
- Obtains and compiles current, detailed, and complete home medication history lists, including drug names, doses, frequencies, and routes of administration.
- Compiles patient allergy information, including reactions experienced by the patient.
- Uses various sources to gather the most current home medication history and allergy information, such as patient and family interviews, outpatient pharmacies, extended care facilities, and provider offices.
- Communicates discrepancies in the medication history process with providers and pharmacists, ensuring unresolved issues are tracked and resolved.
- Refers suspected medication errors to pharmacists for appropriate follow-up and reporting.
- Serves as a resource to pharmacists, providing additional information and following up as needed.
- Documents detailed and verified home medication lists and allergy information accurately in the electronic medical record (EMR).
- Ensures patient allergy information is correctly documented in the EMR and updated as necessary.
- Manages and responds to requests for medication histories, ensuring timely follow-up and resolution.
- Provides high-quality customer service during interactions with patients, families, and healthcare providers.
- Participates in quality assurance and improvement activities related to the pharmacy-managed medication reconciliation practice model.
- Utilizes experience to identify areas for improvement in quality, safety, regulatory, and financial outcomes.
- Rotates through all applicable medication history work assignments and shifts.
Minimum Qualifications
- High school graduate or equivalent degree.
- Maintains current Rhode Island Pharmacy Technician licensure.
- Holds and maintains National Pharmacy Technician Certification (CPhT), or has at least one year of licensed pharmacy technician experience in an outpatient pharmacy setting, and plans to sit for the CPhT exam within six months of hire, with certification within twelve months.
- One to two years of pharmacy technician experience in an outpatient pharmacy setting.
- Strong customer service presence and verbal/written communication skills.
- Excellent focused and active listening skills.
- Proficient in computer skills, including Microsoft Office Suite, email, and calendar applications, and familiarity with internet web applications and pharmacy/system-wide applications.
- Ability to pay close attention to detail and maintain high accuracy and customer service standards.
Work Environment and Physical Requirements
- Based in the Emergency Department or Admitting Department initially.
- Requires ability to focus in a fast-paced environment.
- May involve extended periods of standing and walking.
- Requires visual acuity and manual dexterity for operating a computer and navigating screens.
- New hires must pass a drug screen in compliance with Rhode Island state law.
Independent Action
- Performs independently within department policies and practices.
- Refers complex problems to supervisor for clarification of departmental policies and procedures.
