About the role

NHC HomeCare Laurens is seeking a Medical Social Worker to join their team. The Medical Social Worker supervises and provides evaluations of the social and emotional needs of patients related to their medical conditions, offering counseling and other support.

Responsibilities

  • Assesses social and emotional factors to minimize their impact on the patient’s health and ability to cope with daily living issues.
  • Helps patients and caregivers understand, accept, and follow medical recommendations.
  • Aids patients and caregivers in addressing personal and environmental challenges that may lead to illness or hinder access to healthcare.
  • Identifies and connects patients with community resources and supports them in resuming life in the community, including discharge planning.
  • Collaborates with healthcare providers and home care team members to understand significant social and emotional factors impacting health problems and participates in developing the plan of care.

Requirements

  • Master’s Degree in Social Work from an accredited school.
  • One year of social work experience in a healthcare setting.
  • State-specific requirements:
    • Licensed by the South Carolina Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.

Qualifications

  • Comprehensive knowledge of social work principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong problem-solving and critical thinking skills.
