About the role

NHC HomeCare Laurens is seeking a Medical Social Worker to join their team. This position supervises and provides evaluations of the social and emotional needs of patients related to their medical conditions, offering counseling and other support based on these evaluations.

Responsibilities

  • Restores patients to optimal social and health adjustment through direct social services.
  • Affirms and follows medical recommendations for patients and caregivers.
  • Assists patients and caregivers with personal and environmental issues that may lead to illness or hinder medical benefits.
  • Identifies and connects patients and caregivers with community resources and agencies to facilitate community reintegration, including discharge planning.
  • Collaborates with healthcare providers and home care teams to understand and address significant social and emotional factors impacting health problems.
  • Contributes to the development of the patient’s care plan.

Requirements

  • Masters Degree in Social Work from an accredited institution.
  • One year of experience in a healthcare setting.
  • License by the South Carolina Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.

Qualifications

  • Master’s Degree from a school of social work accredited by the Council on Social Work Education.
  • One year social work experience in a health care setting is required.
  • State-specific requirements must be met, including licensure by the South Carolina Board of Social Work Examiners.
