Medical Social Worker - Part-Time (PRN)
Position Purpose
We are seeking a compassionate and experienced Medical Social Worker to join our team on a per diem basis, providing coverage as needed and supporting patients and families through critical moments of care and transition.
Key Responsibilities
Care Coordination & Case Management
- Collaborate with physicians, nurses, case managers, and care teams to support high-risk patients
- Provide education to patients and families on insurance and regulatory processes (AHCCCS, Medicare)
- Serve as a liaison between patients, families, and healthcare teams
- Maintain accurate and timely documentation
Psychosocial Support & Intervention
- Assess psychosocial needs, coping abilities, and support systems
- Connect patients and families to community resources and financial assistance
- Identify and address concerns such as abuse, neglect, and mental health needs
- Advocate for patient autonomy and informed decision-making
Discharge Planning
- Coordinate discharge plans for complex patients
- Facilitate transitions to post-acute care and community services
- Ensure compliance with payer and regulatory requirements
Patient & Family Support
- Provide crisis intervention and support (e.g., abuse, domestic violence, end-of-life decisions)
- Report suspected abuse to appropriate agencies
- Aid in guardianship, advance directives, and care planning
- Provide grief counseling and emotional support
- Assist with grief support groups and community education initiatives
Compliance & Professional Development
- Adhere to all legal, ethical, and regulatory standards
- Participate in interdisciplinary collaboration and quality initiatives
- Maintain ongoing professional development
