Medical Social Worker (MSW) – Home Health Full-Time or Part-Time

ABL Home Health is dedicated to providing high-quality, compassionate care in patients' homes. We seek a Medical Social Worker (MSW) to support patients and their families in Palo Alto, CA.

Responsibilities

• Assess the psychosocial needs of patients and families to develop individualized care plans.
• Provide counseling and emotional support to patients and caregivers.
• Aid in connecting patients and families to community resources, financial assistance, and legal aid.
• Collaborate with nurses, therapists, and physicians to ensure comprehensive care planning.
• Document all services provided in compliance with agency policies and regulatory requirements.

Requirements

• Master's degree in Social Work (MSW) – Required
• 2+ years of MSW experience in a Home Health or similar healthcare setting – Required
• Strong interpersonal, organizational, and counseling skills
• Valid Driver's License and reliable transportation
