Medical Social Worker (MSW) – Home Health Full-Time or Part-Time

ABL Home Health is dedicated to providing high-quality, compassionate care in patients' homes. We seek a Medical Social Worker (MSW) to support patients and their families in Sunnyvale, CA.

Responsibilities

• Assess psychosocial needs and develop individualized care plans
• Provide counseling and emotional support to patients and caregivers
• Connect patients and families to community resources, financial assistance, and legal aid
• Collaborate with nurses, therapists, and physicians for comprehensive care planning
• Document services provided in compliance with agency policies and regulations

Requirements

• Master's degree in Social Work (MSW)
• 2+ years of MSW experience in Home Health or similar healthcare settings
• Strong interpersonal, organizational, and counseling skills
• Valid Driver's License and reliable transportation
