Compensation

$30.58 – $45.86 per hour, depending on experience

Responsibilities

• Conduct psychosocial assessments of patients.
• Develop individualized care plans based on patient needs.
• Provide counseling and support to patients and families.
• Coordinate with healthcare teams to ensure comprehensive patient care.
• Advocate for patients’ needs and rights within the healthcare system.
• Assist patients in navigating the healthcare system and accessing resources.
• Contribute to discharge planning and continuity of care for patients.
• Provide crisis intervention and emergency support.
• Facilitate guardianship applications and court hearings.
• Organize and lead support groups for patients and families.
• Educate patients and families about healthcare options and resources.
• Collaborate with community organizations to connect patients with additional services.
• Participate in multi‑disciplinary team meetings to discuss patient care.
• Assist patients with financial concerns and connect them to relevant resources.
• Provide bereavement counseling and support.
• Document all patient interactions and treatment sessions in compliance with relevant regulations and organization policies.
• Work collaboratively within a multi‑disciplinary treatment team.
• Adhere to all state and federal regulations surrounding HIPAA/Title 42 requirements and maintain strict confidentiality.
• Serve as a mandated reporter and follow all legal and ethical guidelines.
• Participate in clinical supervision in accordance with NYSDOE regulations.
• Maintain licensure throughout the entirety of employment.

Qualifications

• Minimum Required: Master’s degree in social work.
• New York State Licensed Master Social Worker (LMSW). Limited Permit holders must obtain full licensure within one year.
• Preferred: 3 years post‑graduate experience.
