Compensation
$30.58 – $45.86 per hour, depending on experience
Responsibilities
- Conduct psychosocial assessments of patients.
- Develop individualized care plans based on patient needs.
- Provide counseling and support to patients and families.
- Coordinate with healthcare teams to ensure comprehensive patient care.
- Advocate for patients’ needs and rights within the healthcare system.
- Assist patients in navigating the healthcare system and accessing resources.
- Contribute to discharge planning and continuity of care for patients.
- Provide crisis intervention and emergency support.
- Facilitate guardianship applications and court hearings.
- Organize and lead support groups for patients and families.
- Educate patients and families about healthcare options and resources.
- Collaborate with community organizations to connect patients with additional services.
- Participate in multi‑disciplinary team meetings to discuss patient care.
- Assist patients with financial concerns and connect them to relevant resources.
- Provide bereavement counseling and support.
- Document all patient interactions and treatment sessions in compliance with relevant regulations and organization policies.
- Work collaboratively within a multi‑disciplinary treatment team.
- Adhere to all state and federal regulations surrounding HIPAA/Title 42 requirements and maintain strict confidentiality.
- Serve as a mandated reporter and follow all legal and ethical guidelines.
- Participate in clinical supervision in accordance with NYSDOE regulations.
- Maintain licensure throughout the entirety of employment.
Qualifications
- Minimum Required: Master’s degree in social work.
- New York State Licensed Master Social Worker (LMSW). Limited Permit holders must obtain full licensure within one year.
- Preferred: 3 years post‑graduate experience.
