Medical Social Worker - LMSW

Responsibilities

• Affords assistance to the patient and family with health-related financial, social, and emotional concerns.
• Provides social work services in line with the plan of care.
• Ensures timely communication of changes in the patient's clinical status to other healthcare team members and care center office staff.
• Educates the patient and family members and other team members about medical social work interventions and treatment plans.
• Performs care planning, transfer, and discharge functions.
• Identifies risks or threats to the patient's health in their environment and implements appropriate interventions to mitigate these risks.
• Communicates identified risks/threats to the physician and other members of the healthcare team to coordinate care effectively.
• Performs other duties as assigned.

Qualifications

• Holds a master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education.
• Has one year of social work experience in a healthcare setting.
• Maintains compliance with state and federal requirements for this position.
• Carries current CPR certification.
