Medical Social Worker - LMSW

Responsibilities

• Affords assistance to the patient and family with health-related financial, social, and emotional concerns.
• Provides social work services in alignment with the patient’s care plan.
• Ensures timely communication of changes in the patient’s clinical status to other healthcare team members and care center office staff.
• Educates the patient and family members, as well as other team members, on medical social work interventions and treatment plans.
• Performs care planning, transfers, and discharges.
• Identifies risks or threats to the patient’s health and implements appropriate interventions to mitigate these risks.
• Communicates identified risks/threats to the physician and other members of the healthcare team to coordinate care effectively.

Qualifications

• Holds a master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education.
• Has one year of social work experience in a healthcare setting.
• Mets state and federal requirements for this position.
• Currently holds a valid CPR certification.

Pay

$50 - $66 per hour.

Schedule

PRN (as needed).
