Job Summary

Up to $20,000 Sign-on Bonus Based on Eligibility

Responsibilities

- Identifies patients in need of social work intervention within 24 to 48 hours of admission based on high risk screening criteria. - Follows up on all referrals made to social work from all sources other than high risk screening within 24 hours of referral. - Assesses patients to determine the bio-psychosocial factors that impact their medical condition, treatment, recovery, and safe transition from one care environment to another - Develops intervention plan and discharge plan to meet patient’s/family’s needs. - Provides social work services to patients in OB and takes weekend on call for OB/emergency department. - Other duties as assigned.

Qualifications

- Required: - Education: Master’s degree in Social Work - Experience: One year experience in an acute inpatient hospital setting or equivalent - Licensure/Certification: LCSW eligible. - Preferred: - Licensure/Certification: LICSW preferred.
