Medical Social Worker, Hospice, Full time

About the Role

BAYADA Home Health Care is seeking an energetic and experienced Licensed Medical Social Worker (MSW) for a full-time opportunity performing hospice visits in the Norwich, VT hospice office. This role serves clients on a per-visit basis in territories throughout Windsor, Orange, and Grafton, NH counties, primarily between St. Johnsbury and Ludlow, VT. Prior home care experience is preferred but not required.

As a Medical Social Worker, you will be an integral member of a multi-disciplinary healthcare team providing skilled nursing and palliative care, enabling clients to receive necessary medical care while remaining at home.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assist the team in understanding the social and emotional factors related to clients’ health problems.
• Provide clinical social evaluations and plan appropriate interventions based on findings.
• Identify and refer clients and families to appropriate community resources to address practical and environmental needs.
• Act as an advocate for clients and families to navigate community systems.
• Participate in developing the total plan of care with the multidisciplinary team.
• Develop and maintain contracts with public and private agencies as resources for clients.
• Educate patients, family members, or representatives on community resources to promote health, safety, well-being, and independent living.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.

Qualifications

• Minimum of a Master’s in Social Work, with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education (demonstrated by school transcript or degree).
• Home care experience is a plus.
• Demonstrated ability to read, write, and communicate effectively in English.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to assigned cases.

Pay

Base pay ranges from $36 to $43 per hour, depending on qualifications and subject to negotiation.
