About the role

BAYADA Home Health Care is seeking a Medical Social Worker, MSW, for a full-time hospice visiting position in Norwich, Vermont. The office serves clients across Windsor, Orange, VT and Grafton NH counties. The role involves making home visits, assisting with social and emotional factors, providing clinical evaluations, and advocating for clients.

Responsibilities

  • Making home visits to clients in designated territories.
  • Aiding the team in understanding the social and emotional aspects of clients' health issues.
  • Conducting clinical social evaluations and planning interventions based on evaluation findings.
  • Identifying and referring clients and families to appropriate community resources.
  • Participating in the development of the total plan of care with the multidisciplinary team.
  • Negotiating with public and private agencies for patient resources.
  • Educating patients, family members, or other representatives about community resources.
  • Accurately documenting observations, interventions, and evaluations using a touchpad tablet.

Qualifications

  • A minimum of a Master's in Social Work with at least 2 years of community-based experience.
  • Accreditation by the Council on Social Work Education.
  • Prior home care experience is preferred but not required.
  • Strong communication and interpersonal skills.
  • Proficient in reading, writing, and English.
  • Ability to work independently and manage time effectively.
  • Strong computer skills, especially with electronic medical records (EMR).

Pay

Base Pay, depending on qualifications and subject to negotiation: $36-$43 per hour.

Schedule
