About the role

The Medical Social Worker collaborates with patients, families, caregivers and staff across all disciplines and departments. Responsibilities include providing psychosocial assistance, conducting assessments, and coordinating services for individuals with terminal, acute, and chronic illnesses.

Responsibilities

  • Supports the Central Health Medical Management department with case management function, emphasizing psychosocial, behavioral health, and substance abuse service coordination.
  • Develops professional relationships with aftercare resources and their referral processes to ensure Member safety in the community.
  • Conducts Psychosocial Assessments to share information with other multi-disciplinary members, enhancing the effectiveness and outcomes of the plan-of-care.
  • Identifies barriers to health maintenance and risks of psychiatric diagnoses or substance abuse, making appropriate recommendations and referrals to community resources.
  • Prevents communication breakdowns between members, families, and providers, balancing advocacy and staff roles.
  • Identifies signs of abuse, neglect, exploitation, or domestic violence and reports to designated agencies.
  • Provides support to members/families facing illness and psychosocial issues.
  • Addresses causes of frequent medical and/or behavioral health re-admissions within the scope of Social Work.
  • Interacts independently with healthcare professionals, demonstrating critical thinking and initiative.
  • Handles confidential and time-sensitive materials, adhering to HIPAA regulations.
  • Uses computers for word processing, data entry, database creation, online research, letter formatting, report composition, and composing routine correspondence.
  • Performs job duties accurately and promptly.
  • Performs other tasks as assigned.

Knowledge/Skills/Abilities

  • Working knowledge of community agencies/resources and financial programs like TANF/SSI.
  • Experience with crisis intervention with adult populations.
  • Ability to work with multidisciplinary teams.
  • Previous experience in a managed care environment.
  • Participates in maintaining a positive team environment focused on customer satisfaction.
  • Organizes work and manages time efficiently, finding innovative ways to improve efficiency.
  • Handles sensitive and confidential communications and materials with discretion and judgment.
  • Manages multiple priorities effectively and communicates clearly verbally and in writing.
  • Uses Microsoft Office Suite and internet skills.
  • Bilingual skills (English and Spanish) are highly desirable.

Minimum Education

A Bachelor’s Degree in Social Work from an accredited school of Social Work is required.

Minimum Experience

A minimum of three years of experience in medical social work with vulnerable populations is required.

Required Certifications/Licenses

Holds and maintains the following certifications/licenses:

  • Licensed Bachelor Social Worker (LBSW)
  • Licensed Master Social Worker (LMSW)
  • Licensed Clinical Social Worker (LCSW)
