About the role
The Social Worker (BSW) assists patients and families with complex medical and behavioral health needs to navigate through the course of their illness, aiming to improve or maintain social, emotional, functional, and physical health. Practices professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work. Must maintain a current Ohio Social Work license.
Responsibilities
- Functions as a member of an interdisciplinary team and provides care coordination services to patients and their families, including:
- Screening and identification of patients.
- Outreach to primary care physicians, specialty physicians, and other service providers to identify patients in need of care coordination.
- Assisting Care Coordinator in completion of Health Risk Assessment.
- Assisting Care Coordinator with the development of a family-centered plan of care.
- Facilitating communication and collaboration amongst the healthcare team.
- Developing patient’s/family self-management skills through education and resource provision.
- Performing ad hoc tasks as directed by Care Coordinator.
- Managing transitions between settings, caregivers, and providers.
- Monitoring and follow-up across the continuum of care.
- Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal healthcare services.
- Participates in orientation and continuing education of staff and students as appropriate.
Requirements
- BSW, required.
- LSW (Licensed Social Worker), required.
- Valid Ohio driver’s license and proof of auto insurance as required by hospital policy.
- Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.
- Active BLS (Basic Life Support) certification, required.

- Computer literate with working knowledge of Microsoft products (e.g., Outlook, Word, PowerPoint, Excel, Teams).
- Working knowledge of Medicaid and other regulatory agency standards.
- Basic knowledge of healthcare terminology.
Qualifications
- Experience working in healthcare in a patient-facing role, preferred.
- Electronic Health Record (EHR) experience, required.
- Working knowledge of Medicaid and other regulatory agency standards, required.
Schedule
40 hours per week, Monday through Friday, 8:00 AM – 5:00 PM.
