Medical Social Worker

About the role

NHC HomeCare Konxville is seeking a Medical Social Worker to join their team. This position supervises and provides evaluations of social and emotional needs related to medical conditions, offering counseling and other support.

Responsibilities

• Restores patients to optimal social and health adjustment by providing and documenting direct social services.
• Affirms social and emotional factors to mitigate their impact on health and daily living.
• Supports patients and caregivers in understanding and following medical recommendations.
• Assists with personal and environmental issues that may lead to illness or hinder medical care, and identifies community resources to help patients return to community life, including discharge planning.
• Collaborates with providers and home care team members to understand significant social and emotional factors affecting health problems, and participates in developing the plan of care.

Requirements

• Masters Degree in Social Work from an accredited school.
• One year of social work experience in a healthcare setting.
• Tennessee specific: Current state certification as a Certified Master Social Worker (CMSW) or current license as a Licensed Clinical Social Worker (LCSW).

Qualifications
