Position Overview
The medical social worker provides assessment, evaluation, and referral to services for Home Health patients within the greater Monterey and Salinas County areas. Essential Job Functions:
- Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
- Carrries out social evaluations and plans intervention based on evaluation findings.
- Maintains clinical records on all patients referred to social work.
- Provides information and referral services for patients and families/caregivers regarding practical and environmental needs.
- Serves as liaison between patients or families/caregivers and community agencies.
- Maintains collaborative relationships with organization personnel to support patient care.
- Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
- Participates in the development of the total plan of care and case conferences as required.
- Aids physician and other team members in understanding significant social and emotional factors related to health problems.
- Assists with discharge planning.
Basic Qualifications
- Master’s degree in social work accredited by the Council on Social Work Education.
- One (1) year experience as a Medical Social Worker (MSW).
- One (1) year experience working with patients in Home Health.
- Demonstrates good verbal and written communication, and organization skills.
- Current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Physical Requirements
- Typical functions include standing, sitting, and walking a majority of the time.
- Occasions may require kneeling, stooping, bending, or reaching.
- Lifts and/or moves up to 10 lbs on a regular basis.
- May need to lift or move up to 40 lbs occasionally.
- Requires typical vision abilities including close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
