Medical Social Worker

Position Overview

The medical social worker provides assessment, evaluation, and referral to services for Home Health patients within the greater Monterey and Salinas County areas. Essential Job Functions:

  • Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
  • Carrries out social evaluations and plans intervention based on evaluation findings.
  • Maintains clinical records on all patients referred to social work.
  • Provides information and referral services for patients and families/caregivers regarding practical and environmental needs.
  • Serves as liaison between patients or families/caregivers and community agencies.
  • Maintains collaborative relationships with organization personnel to support patient care.
  • Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
  • Participates in the development of the total plan of care and case conferences as required.
  • Aids physician and other team members in understanding significant social and emotional factors related to health problems.
  • Assists with discharge planning.

Basic Qualifications

  • Master’s degree in social work accredited by the Council on Social Work Education.
  • One (1) year experience as a Medical Social Worker (MSW).
  • One (1) year experience working with patients in Home Health.
  • Demonstrates good verbal and written communication, and organization skills.
  • Current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Physical Requirements

  • Typical functions include standing, sitting, and walking a majority of the time.
  • Occasions may require kneeling, stooping, bending, or reaching.
  • Lifts and/or moves up to 10 lbs on a regular basis.
  • May need to lift or move up to 40 lbs occasionally.
  • Requires typical vision abilities including close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
