Medical Social Work Hospice

Amedisys medical social worker position covering a 30-mile radius of Leominster, offering part-time day shift with flexible, purpose-driven work to help patients heal at home.

Pay

$39.00 - $42.00 per hour.

Responsibilities

• Assists the patient and family with health-related financial, social, and emotional concerns and teaches effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures timely communication of changes in the patient's clinical status to healthcare team members and care center office staff.
• Educates the patient, family members, and team members on medical social work interventions and treatment plans.
• Performs care planning, transfer, and discharge functions.
• Identifies risks or threats to the patient's health in their environment and promptly implements interventions to reduce risks.
• Communicates identified risks/threats to the physician and healthcare team to coordinate care effectively.
• Other duties as assigned.

Qualifications

• Master’s or doctorate degree from a school of social work accredited by the Council on Social Work Education.
• One year of social work experience in a healthcare setting.
• Meets state and federal requirements for the position.
• Current CPR certification.
