Medical Director/Attending Physician - Post-Acute Care (Full-Time)

We are seeking a full-time Advanced Practice Provider in Toledo, OH to serve local skilled nursing facilities within a close-knit, mission-driven care community. Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities.
- Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
- Communicate with facility nursing staff via phone or secure messaging as needed.
- Maintain reliable on-site attendance for scheduled shifts.
- Ensure compliance with clinical standards, facility policies, and regulatory requirements.
Requirements
- Hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
- Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
- Ability to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.

Pay
The estimated compensation range for this position is $104,000 to $169,000 per year, based on expected earnings under our uncapped productivity-based compensation model.
Schedule
- 6-hour facility shifts for full-time positions (rounds generally start between 7-10 a.m.)
- Freedom from on-call and overnight requirements
