Responsibilities
- Serves as a Clinical Partner supporting the mission, vision, and core values of HarmonyCares.
- Supports patient care through clinical partnership with Nurse Practitioners, Physicians, and Care Management Teams.
- Coordinates daily travel itinerary and transports clinicians to scheduled appointments.
- Prepares patients for examination and treatment; may include preparing patients for minor procedures.
- Sets up patient care as directed by Clinicians, adhering to company standards and policies.
- Documents patient care accurately and in a timely manner.
- Respects patient confidentiality and treats patients with courtesy and respect.
- Performs phlebotomy (venipuncture only), EKG, or other procedures as requested by Clinician.
- Labels, stores, and packages specimens for lab delivery.
- Updates patient care plans, SDoH (Social Determinants of Health) assessments, and other documentation in the patient chart or electronic medical record.
- Maintains compliance with CLIA, OSHA, and HIPAA guidelines.
- Ensures supplies and equipment are prepared and maintained for treatment.
- Ensures the company vehicle is clean and follows a proper maintenance schedule.
- Attends required meetings and in-services.
Qualifications
- High school diploma or GED equivalent.
- Graduate of an accredited Medical Assistant, Phlebotomy, EMT, or related medical education/training program.
- One year of experience as a Medical Assistant or Phlebotomist.
- Current CPR certification meeting Company guidelines.
- Knowledge of medical terminology and basic computer literacy, including EMR, iPad, and GPS.
- Strong interpersonal communication and customer service skills.
- Valid driver's license and good driving record.
- Desire and ability to obtain MA certification within 180 days of hire.
