Responsibilities

  • Care Manager Duties:
    • Responsible for a designated group of residents during the shift; observes, reports, and documents symptoms and conditions of residents.
    • Assists with continence management and proper disposal of continence products.
    • Serves and collects food trays and assists as needed; reports refusal of meals or loss of appetite.
    • Maintains cleanliness of resident’s room and work areas.
    • Engages residents in activities of daily living and life enrichment activities.
    • Greets and assists all internal and external customers, guests, family members, residents, vendors, and team members.
    • Does resident laundry as assigned and needed.
  • Medication Care Manager Duties:
    • Reviews service plan to learn pertinent information about residents.
    • Aids in administering medications and treatments, documenting and initialing forms.
    • Maintains confidentiality of resident information including resident medication.
    • Reports resident concerns to the Nurse and/or Health Care Coordinator.
    • Restocks medication cart after all medication passes.
    • Participates in the development of the Service Plan and monthly updates.
    • Takes and records vital signs such as temperature, blood pressure, weight, pulse, and respiration rates.

Qualifications

  • High School diploma/GED.
  • Previous experience working with seniors preferred.
  • Must be 18 years of age.
  • Certified Nurse’s Aid certification preferred.
  • CPR and First Aid certifications preferred.

  • Effective communication skills.
  • Ability to apply common sense understanding to carry out instructions.
  • Good judgment, problem-solving, and decision-making skills.
  • Interest in working with a senior population.
  • Ability to remain calm in difficult situations.
  • Competent in organizational and time management skills.
