Measure Optimization Lead - Care Coordination

For our client, we are seeking a Measure Optimization Lead - Care Coordination to join the team of a leader in the Healthcare space. This role will drive performance improvement efforts focused on care coordination measures within a highly regulated healthcare environment. You will partner with operational, clinical, and leadership stakeholders to identify barriers, translate data into action, and strengthen outcomes tied to quality and rating performance. The position combines analytical insight, cross-functional influence, and subject matter expertise to help shape strategic decisions and implementation plans.
