About the role

Warsaw Rehabilitation and Healthcare Center, located in Warsaw, NC, is a Long Term Care facility that provides quality care to its residents. They are currently hiring for a Full-Time MDS Coordinator position.

Responsibilities

  • Keep abreast of current federal and state regulations, as well as professional standards.
  • Aid in the development of comprehensive care plans for Residents in coordination with the MDS that accurately address their needs.
  • Coordinate, manage, and monitor the written plan of care for each Resident of the facility, ensuring it identifies the needs and sets goals for each need.
  • Collaborate with nursing management to coordinate, manage, and review nurse's notes to ensure the care plan is being implemented correctly.
  • Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
  • Audit MDS and support documentation to ensure accuracy and compliance with regulatory and auditor requirements.
  • Aid in the discharge process by coordinating with nursing management.
  • Complete necessary administrative tasks, including filling out forms and reports.
  • Maintain strict confidentiality regarding sensitive health information of Residents.
  • Report hazardous conditions, damaged equipment, and supply issues to appropriate personnel.
  • Maintain the comfort, privacy, and dignity of Residents and interact with them in a warm, respectful, and caring manner.
  • Respond promptly and courteously to call lights in Resident care areas.
  • Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.
  • Attend and participate in departmental meetings and in-services as directed.

Requirements

  • Current State License as a Nurse
  • CPR Certification
  • Prior MDS Nurse experience (preferred)
  • One year experience as a Nurse in a long-term care setting (preferred)
