About the role
A MDS Coordinator (RN) position focused on patient assessment rather than direct care, offering a Monday through Friday schedule. The Resident Assessment Coordinator (RAC) leads an interdisciplinary team to conduct comprehensive assessments, ensuring holistic care plans centered on residents' wants, needs, and goals. This role is ideal for a detail-oriented nurse with strong documentation and assessment skills, who employs critical thinking and the nursing process.
Responsibilities
- Coordinate the interdisciplinary assessment process for assigned residents.
- Coordinate development of and ensure completion of Comprehensive Care Plans in accordance with company, state, and federal guidelines.
- Monitor residents to ensure individual Care Plans are followed and updated as necessary.
- Report any variances to the Director of Nursing and Care Plan Team.

Requirements
